Step 1: Creating Engagement Triggers
ClOEE uses preventive service qualifiers and possible care gaps as Engagement Triggers™. Potentially at-risk patients whose clinical measurables match the pre-defined Engagement Triggers™ are automatically enrolled into clinical outreach campaigns.
Each Healthcare organization has its own clinical goals and objectives. With ClOEE organizations can configure numerous Engagement Triggers™ to maximize patient engagement. Out of the box ClOEE offers published USPSTF recommendations as Engagement Triggers™. Organizations can also create custom triggers.
Use Case: Flu Vaccine
A hospital system is preparing for flu season. The system has a large 65+ aged population. Most of the patients have completed their Annual Wellness Visits (AWV). The hospital system would like to aggressively screen patients who have not had an influenza shot in over 12 months. ClOEE is able to process clinical data from the AWV and identify patients who meet the Engagement Trigger™, Age =< 65 yrs, Last Influenza shot > 12 months. ClOEE identifies the at-risk group and automatically launches clinical outreach campaigns, geared at engaging the at-risk population to receive additional screening or get a flu shot.
Step 2: Clinical Data Transmission
Now that we've configured the Engagement Trigger™, how do we get the clinical data into ClOEE? ClOEE’s underlying interoperability infrastructure supports clinical data ambiguity, this allows ClOEE to securely exchange and transmit clinical data between disparate health systems in various data formats.

Step 3: Patient Registration & Enrollment
Patient registration is the process of adding patients to ClOEE. ClOEE supports manual and dynamic patient registration protocols. Manual registration is done via patient list uploads. Dynamic registration allows patient addition via web-service methods, this includes HL7, FHIR & API protocols.
Patient enrollment is the qualification of patients into clinical engagement campaigns. ClOEE’s inbuilt logic engine and algorithms processes inbound clinical data and identifies patients who meet the potential risk threshold of defined Engagement Triggers™.
Step 4: Patient Outreach & Engagement
